Initial Assessment

The initial assessment is critical to developing the right plan of care for your loved one. At the initial assessment we will ask for information about the person we will be caring for, such as:

• A simple biography of their life, including information about their childhood, education, family and life history.
• What activities do they like to participate in?
• What are their likes, dislikes, hobbies and interests?
• What is the health condition, past, present and future?
• If there are physical limitations, is the design of the home set-up to accomodate them?
• Are there other services outside of what Thoughtful Care can provide that we can recommend?
• And most important, what does the person we are caring for want?
